This is a list of items you'll need to provide to the UNM-Taos nursing program with your application.

  Item needed Due date Special instructions
1. Official transcripts 12/15/10 GED, High School or College transcripts. Current grades from all pre-requisites. Computer literacy is required.
2. Current CPR card 12/15/10 American Heart Association/ Basic Life Support CPR Required.
3. TEAS Test Results 12/15/10 Printed copy of test scores.
4. CNA certificate 12/15/10 Certificate or written statement from course instructor of anticipated completion date with testing date.
5. 3 Letters of Reference 12/15/10 A total of 3 letters of reference. At least one should be from a current or former employer and one from an instructor. If there is not an employment history then at least 2 from instructors. Areas to be addressed are: critical thinking, leadership, science, awards/honors.
