1.	Official Transcripts	5/23/14 noon deadline	High School and College transcripts. GED Accepted. Minimum 3.0 GPA required.
2.	Current CPR/BLS Card	5/23/14 noon deadline	Accepted only from the American Heart Association.
3.	TEAS V Test Results (Teas V results must be within one year)   TOEFL test	5/23/14 noon deadline	64 is the minimum total adjusted score benchmark with a minimum of 50% in reading, math, science and English to qualify for application.   TOEFL test if ESL student.
4.	CNA Certificate	5/23/14 noon deadline	Certificate within the last 5 years.
5.	Letter of Intent/Current Resume	5/23/14 noon deadline	Resume needs to include any health care related experience. Letter of Intent and Current Resume.
6.	UNM-Taos Nursing Application Form PDF	5/23/14 noon deadline	Available on line. Submit a hard copy of this form with your application.

Selection Criteria

• 50% TEAS Score - adjusted individual total score will be used, application benchmark is 64
• 35% GPA from pre-requisite math and science courses
• 15% for completion of the co-requisite courses of A&P 11, Psych 220 and English 102 as follows:
  • 5 points A or above
  • 4 points B+
  • 3 points B
  • 2 points C+
  • 1 point  C
• An extra 5 points will be given for a minimum of 500 hours of previous health care experience by a Human Resources Representative within the last 5 years to include::
  • CNA
  • EMT/Paramedic
  • LVN/LPN
  • Behavioral Health Assistant

Please make an appointment with Linda Nowalk, Administrative Assistant at 737-3745 to turn in all materials. Students will be notified of acceptance into the Nursing Program by email in June 2014. Applicants who are not accepted will also be contacted by email regarding their status. Once students confirm their acceptance, a detailed orientation and information packet will be mailed.
